Skilled Nursing
Our nurses are highly-skilled, experienced home care nurses dedicated to providing quality, compassionate home care. Our Registered Nurses (RN) act as the case managers and coordinate the home care program and treatment plan with the entire UHHS team. An individualized, patient-centered care plan is developed after the RN’s initial evaluation. In close collaboration with the patient’s physician, the RN provides skilled nursing services, which may include the following:
- Assessment and monitoring of condition
- IV therapy
- Wound care
- Dressing changes
- Medication administration and instruction
- Blood draws for lab work
- Patient and family education
- Home health aide supervision
- Bowel and bladder management
- Catheter insertion and care
- Resource development
- Telehealth Phone Monitoring Program
