Skilled Nursing
Our nurses are highly-skilled, experienced home care nurses dedicated to providing high-quality, compassionate home care. Our Registered Nurses (RN) act as the case managers and coordinate the total home care program and treatment plan with the entire UHHS team. An individualized, patient-centered care plan is developed after the RN’s initial evaluation. In close collaboration with the patient’s physician, the RN provides skilled nursing services, which may include the following:
- Assessment and monitoring of condition
- IV therapy
- Wound care
- Dressing changes
- Medication administration and instruction
- Blood draws for lab work
- Patient and family education
- Home health aide supervision
- Bowel and bladder management
- Catheter insertion and care
- Resource development
- Telemedicine program for heart failure.
