The goal of our Psychiatric Home Care Program is to promote the patient’s physical, emotional, and spiritual well-being and to prevent rehospitalization. Our certified psychiatric nurse provides psychiatric assessment, medication teaching and monitoring, lab draws, and establishes an individualized plan of care. The nurse utilizes various techniques to aid the patient in improving coping strategies including:
The psychiatric home care nurse works with our social workers to provide a bridge of care with the community that helps to improve the patient’s quality of life at home.
