Heart Failure Program/Telehealth

United Home Health Services’ Heart Failure Program is aimed at improving patient outcomes, improving quality of life, and decreasing hospital emergency visits and readmissions for our congestive heart failure patients. We follow recommended heart failure protocols and guidelines. Our registered dietitian has a very active role in the nutritional counseling of this patient population.

We offer Telehealth in order to better manage this disease on a daily basis. Telehealth can provide daily weight, blood pressure, blood sugar, and symptom monitoring in the patient’s home. One or all of the above measurements can be obtained, depending on the patient’s individual needs.

The results are automatically sent to the UHHS nurse on a daily basis. This information is analyzed and if abnormalities occur, the nurse will contact the physician for appropriate follow up orders. For example, the physician may adjust the patient’s medications or make changes to the treatment plan.

Telehealth encourages individuals to take a more active role in their disease management and reinforces what they need to do to better manage their heart failure.
