Geriatric Care Management Program
Our Private Duty Service Coordinator, Angie Muscat, LMSW, ACSW is a Professional Geriatric Care Manager. She is a member of the National Association of Professional Geriatric Care Managers and is committed to maximizing the independence and autonomy of older adults and people with disabilities. Angie has extensive training and experience in this area and is available to assist families who need help with care giving issues.
Angie is available to help with the following:
- Conduct care-planning assessments to identify problems, eligibility for assistance, and need for services
- Screen, arrange, and monitor in-home help or other services
- Review financial, legal, or medical issues and make referrals to appropriate geriatric specialists
- Act as a liaison to families at a distance, making sure things are going well and alerting families to problems
- Assist with moving an older adult to or from a retirement complex, care home, or nursing home
- Offer counseling and support
- Provide crisis intervention
- Provide consumer education and advocacy
