If you would like to make a referral to UHHS, please contact us at 734-981-8820 or download the Continuing Patient Care Form and fax to 734-981-8828. Please include as much information as you are able however, the most important areas to complete are patient name, date of birth, address & phone number, diagnosis, insurance information, and home health services required. We also need the physician name and phone number.
