Contigent Social Worker Summary

A professional therapist who provides skilled intervention to homebound patients and their families. The Social Worker completes a thorough psycho-social and environmental assessment and performs counseling for long term planning, referrals to community resources, and/or short-term therapy under the direction of the physician and in collaboration with other team members. The Social Worker formulates realistic goals and evaluates the results upon discharge of the patient.

Required Skills

• Knowledge of current social work methods and community resources.
• Skill in the application of social work practice to clients in the home setting.
• Ability to set priorities and problem solve.

Requirements

Education and Experience:

• Master's Degree in Social Work from a school accredited by the Council on Social Work Education.
• Current unrestricted license to practice social work in the state of Michigan.
• One year experience in the practice of social work in a health care setting.
