Community Health Nurse - RN Summary
A professional nurse who provides skilled nursing care to patients in their residence, on an intermittent basis. Seeks to promote the optimal level of well-being for the patient and family, by identifying problem areas and by the application of counter measures to reduce or resolve them. Delivers healthcare by teaching, direct intervention, use of paraprofessionals, referrals to other disciplines, and conferencing with other members of the health care team to formulate realistic goals and utilizing community resources.

Requirements
- Graduate from an accredited school of nursing, preferably B.S.N.
- Current Michigan license to practice nursing.
- One year work experience in Medical / Surgical Nursing.
