JOB TITLE: Home Health Aide Coordinator

REPORTS TO: Clinical Director

JOB SUMMARY: A professional nurse responsible for the overall coordination and supervision of all paraprofessional staff and promotes an optimal level of well-being for the patient and family through home health aide services.

QUALIFICATIONS:

  • Knowledge, Skills, and Abilities:
    • All qualifications in Community Health Nurse job description.
    • Knowledge of current HHA regulations as outlined in the OBRA act of 1987.
    • Minimum 1 year supervision required.
  • Education and Experience:
    • Graduate of an accredited school of Nursing, preferably BSN.
    • Current Michigan license to practice nursing. Minimum two years nursing experience, at least one year of home health care.

ESSENTIAL JOB FUNCTIONS:

  • Interviews, recommends for hire, and orients paraprofessionals.
  • Plans and implements the HHA competency evaluation program as outlined in the OBRA Act of 1987.
  • Performs and documents introductory and supervisory visits of paraprofessional staff, as well as pertinent information from the paraprofessionals and/or patient/family.
  • Instructs and demonstrates to the HHA staff, new procedures and techniques, in order to assure the delivery of safe competent care to the patient based upon the physician's orders.
  • Determines need for frequency of home visits depending upon staff/patient needs along with interaction of CHN/ Therapist.
  • Reviews, updates, and approves HHA care plan along with interaction of CHN/ Therapist.
  • Participates in Orientation of CHN/Therapist.
