JOB TITLE: Community Health Nurse (CHN) - RN

JOB SUMMARY: A professional nurse who provides skilled nursing care to patients in their residence, on an intermittent basis. Seeks to promote the optimal level of well-being for the patient and family, by identifying problem areas and by the application of counter measures to reduce or resolve them. Delivers healthcare by teaching, direct intervention, use of paraprofessionals, referrals to other disciplines, and conferencing with other members of the health care team to formulate realistic goals and utilizing community resources.

Knowledge, Skills and Abilities:
  • Willingness to understand, support and implement in practice, the philosophy, objectives and policies of the Agency.
  • Affirms a philosophy of nursing which stresses nursing process, professional accountability and professional growth.
  • Knowledge of current professional nursing practice applicable to the care of assigned patients.
  • Skill in the application of nursing process and current nursing techniques to meet patient and family needs.
  • Ability to set priorities and problem solve.
  • Ability to maintain confidentiality.
Education and Experience:
  • Graduate from an accredited school of nursing, preferably B.S.N.
  • Current Michigan license to practice nursing.
  • One year work experience in Medical / Surgical Nursing.
