JOB TITLE: Community Health Nurse - LPN

JOB SUMMARY: The licensed practical nurse functions under the direction and guidance of a registered nurse to carry out the nursing process which includes assessment, planning, implementation and evaluation of patients and Para-professionals in a home setting. Ensures quality and safe delivery of nursing care services.

Knowledge, Skills and Abilities:
  • Willingness to understand, support and implement in practice, the philosophy, objectives and policies of the Agency.
  • Affirms a philosophy of nursing which stresses nursing process, professional accountability and professional growth.
  • Knowledge of current professional nursing practice applicable to the care of assigned patients.
  • Skill in the application of nursing process and current nursing techniques to meet patient and family needs.
  • Ability to set priorities and problem solve.
Education and Experience:
  • Graduate from an accredited school of practical nursing.
  • Current unrestricted Michigan License to practice nursing.
  • One year work experience in Medical-Surgical Nursing.
  • Certificate of successful completion of medication course.
